The Sugar-Free Lie

I ate “sugar-free” chocolate. My glucose monitor didn’t get the memo.

I read the label.

I bought a bag of sugar-free chocolate from the pharmacy aisle last week. The front said “sugar-free” in capital letters. I put on my monitor and ate two pieces on an empty stomach. This is what I do. I test the stuff they tell us is safe.

Thirty-five minutes later, my blood sugar had climbed 35 points. That is more than half the spike I get from a regular candy bar. And this bag promised me nothing would happen.

The ingredient list told the whole story. Second line: maltitol syrup. This is a sugar alcohol, and food companies use it because they can stamp “sugar-free” on the front while maltitol raises your glucose from the inside. It tastes like sugar. It bakes like sugar. And your body treats it a lot more like sugar than the label wants you to believe.

Maltitol powder has a glycemic index of 35. Its syrup form (cheaper, found in most budget brands) scores 52. Regular table sugar sits at 65. In other words, the syrup gets you 80% of the way to real sugar, and the package still says “sugar-free.”

So how is this legal? Because maltitol is not technically a sugar. Under FDA rules, sugar alcohols do not count as “sugars” on the nutrition panel. A product can contain maltitol and still carry a sugar-free claim. The fine print is correct. But your pancreas does not read fine print.

Nobody told me this.

I spent two years reaching for those bags in the pharmacy aisle. I thought I was making the smart choice. And the whole time... maltitol syrup was doing half the damage that sugar would have done. Not zero. Half.

Meanwhile... The Hack

Not every sugar-free candy plays this game. Some brands use erythritol, allulose, or monk fruit as the sweetener. Those three have a glycemic index of... zero. I tested an erythritol-based chocolate bar the next morning. Same time. Same empty stomach. Same two pieces.

My blood sugar rose 4 points. That is background noise.

The difference is how your gut handles each one. Your small intestine absorbs about 40% of maltitol and converts part of it into glucose. Erythritol takes a different route. About 90% gets absorbed into your bloodstream, travels through your body, and leaves through your kidneys without ever turning into fuel. In other words, it passes right through. Maltitol does not.

I went back to the store and checked six bags in the sugar-free section. Four of them used maltitol or maltitol syrup. Two used erythritol. The price gap was about a dollar. The glucose gap was 31 points.

🔬 Where this comes from: Maltitol powder has a glycemic index of 35; maltitol syrup scores 52 (International Tables of GI/GL, 2021). Erythritol has a GI of 0 and does not affect blood glucose or insulin (European Journal of Clinical Nutrition, 2003). A 2014 crossover trial in 36 subjects confirmed that maltitol-containing products produce a measurable postprandial glucose and insulin response compared to control (Respondek et al., EJCN 2014). The sugar-free confectionery market is projected at $2.4 billion in 2026, growing 6.2% annually. Responses may vary by individual.
